Office Hours
Regular business hours are 8:30 am – 5:00 pm. Click here for more information on scheduling appointments.
Telephone Calls
During regular hours, nurses are available to return phone calls and offer advice regarding your child(ren). Please allow 1-1½ hours for the nurse to return your call. We ask that you leave only one number where you can be reached. We will not accept pager numbers. There are instances when we will need to examine your child and the nurse will instruct you if that is the case and offer an appointment. Prescription refills are handled by the nurses. Please call and have the name, strength and dosage of the medication as well as the pharmacy number. Routine prescription refills are only taken during regular office hours.
Most information of a routine nature, such as immunization questions, blue slips and school forms may be handled by our receptionists. Please call for this information during regular hours.
After-Hours Telephone Calls
Nurses are available to return phone calls regarding sick children until 8:30 pm on weekdays and 6:00 pm on weekends. Please do not call for routine questions, formula changes, prescription refills and medication changes.
Forms
Sports physicals, school forms, medication forms, WIC forms, etc.
These forms may be brought with your child and filled out at the time of the well child exam. If not, we ask that you drop them off at the front desk with a phone number where you can be reached if there are any questions. If your child has not had a current physical, one will be need to be scheduled prior to the physician filling out and signing the form. For medication forms, please fill in the name of the medication you are asking us to authorize your child taking. Please allow at least 24 business hours before returning to pick the form up. Due to HIPPA regulations, we are not authorized to fax any forms.
